Handrails and balustrades are the safety hardware people actually put their hands on. They get judged on how solid they feel, and tested occasionally by somebody's full weight.
The substrate decides everything
A balustrade is engineered to resist a person falling against it, and it passes that load into whatever it is fixed to. Fixed into sound concrete or steel, it does its job. Fixed into rendered masonry that has softened, a deteriorated concrete edge, or timber with rot at the fixing point, it is decoration.
On older buildings, and particularly on strata blocks where the balcony edge has started to spall, checking and repairing the substrate usually turns out to be a bigger part of the job than the rail itself. Because we do concrete repair as well, we can deal with a failed balcony edge and reinstate the balustrade as one job instead of coordinating two trades who each blame the other.

Compliance requirements for heights, load resistance, openings and climbability come from the Building Code and the relevant standards, and they are the designer's and certifier's call. We install to the specified detail, and we check what we are fixing into before we drill it.
